Important Forms For Referees

Game reports with misconduct and red card resports MUST be submitted to the PYSA office within 24 hours of the match.
 
Red Card Report    
File a report with the PYSA office for every red card
 
Referee Pay Voucher    
For team no-shows and forfeits
 
Form W-9    
Every referee working in PYSA must have a W-9 on file with the league
